Transition Services

At age fourteen (14), a statement of needed transition services will be addressed by the Individualized Education Program (IEP) team, including the student.  A plan for needed transition services will be developed for one or more of the following areas:  academic instruction, community experiences, development of employment, and other post school adult living objectives, acquisition of daily living skills, functional vocational evaluation, or other skills the IEP team determines appropriate.  Every student with disabilities will be referred to a vocational rehabilitation counselor by the age of 14.  The vocational rehabilitation counselor will determine eligibility for services.
