Residency Verification

RESIDENCY VERIFICATION

Residency Verfication must be completed each year for all new and returning students.  The verification process will be completed within the Existing Student/Annual Update through the Parent Portal or within the New Student Application.  Please see the 2023-2024 School Year Registration tab for more information on how to complete the registration.

Once within the application, you will complete all questions and upload your 2 proofs of residency via a Google Form link.  If you were unable to complete the Residency Verification within the online application,please use the link below to access.

You will need the following documents in order to complete the Residency Verification:

  •  (2) Proofs of residency, choose from list below:

    • Must be current bills servicing the home within 30 days, (Gas, Water, Electric, Trash) – Provider Name must be visible!

    • The bills must include parent/guardian(s) name and THE SERVICE ADDRESS OF THE PROPERTY .  NOT THE REMITTANCE / MAILING PORTION

    • Builder contract/Residential Sale – Must include the property address, closing date within 90 days and signatures from all parties

    • Lease/Rental Agreement – Must include the property address, parent/guardian names and Landlord names, lease/rent term dates and signatures from all parties

    • Mortgage Statement – Must include the parent/guardian names and the property description address

    • If you are a parent/guardian that is living with family/friends and can not provide this information in your name, you will be required to complete a Residency Affidavit which the homeowner or person you are living with must sign and provide the (2) bills required above.  Please come directly to the Administration office after completing the online registration if this is your living situation or call 405-381-2605 for more information.

    • All documents uploaded must be a .jpg or .pdf document

    • Documents not accepted will be credit card bills, cut-off notices, past due bills.  Documents must be current, must have homeowners name (parent/guardian) and must show the service address/service decription address.

FAILURE TO PROVIDE THE REQUIRED DOCUMENTS AND INFORMATION REQUESTED ABOVE, YOUR APPLICATION WILL BE PUT ON HOLD AND WILL DELAY THE START OF SCHOOL FOR YOUR STUDENT.
